NOTES TO REMEMBER

  1. The LGFA Injury Fund IS NOT INSURANCE! Please see page 1 of the Injury Fund Guide.
  2. All injuries must be notified within 8 weeks of the date of injury.
  3. All treatment outside what is automatically covered (scans, consultations, surgeries, additional physio etc.) requires prior approval. Prior approval is issued for each treatment requested – if a scan/consultation is approved that approval doesn’t extend to any further treatment undertaken.
  4. Follow up appointments at private A&E clinics require prior approval, as does any consultation with a private concussion clinic.
  5. All terms and conditions of the LGFA Injury Fund are available in the Injury Fund Guide (attached).
  6. Preliminary claim forms and prior approval requests should be submitted by email. Once submitted by email there is no requirement for the documents to be posted. If a preliminary claim form request is posted and it is not received before the 8 week deadline the claim will be
  7. Only the Full Injury Claim Form and medical expense documents or loss of wages documents are required as originals and must be posted. 
  8. All forms must be signed by the club and county boards before being submitted to Croke Park.



REMINDERS - To Note


  • To submit the full injury claim form within the 8 week notification deadline.
  • Players/clubs must sign the full Injury Claim Form in sufficient time for the forms to be posted to county board or Croke Park.
  • Players must be registered at the time of the Injury.
  • Any subsequent treatments must receive prior approval/notification before proceeding.
